On this page you’ll find links to all forms required for children’s files as well as our handbooks and policies.

Required Forms for All Children:

• Emergency Contact Information

• Developmental History Form

• Permissions: Sunscreen, Field Trips, Pet Policy, Hand Sanitizer, and Photo Permissions

• Family Volunteer Form

• Holiday Questionnaire

• Tuition Express Automatic Withdrawal Form (if you prefer not to submit this information electronically, please call or stop by the office to enroll)

• USDA Food Program Form in English or Spanish, and additional languages available here. NOTE: You can leave the income information and SSN in Part 3 blank, unless your family may qualify for free/reduced meals (see USDA guidelines for more information).

Immunization Records:

• PREFERRED: Print your Child's Immunization Record from MyIRmobile - be sure to select the option called “WA Certificate of Immunization Status (CIS) form for school and child care”

• Certificate of Immunization Status - Blank (blank form must be signed by healthcare provider, or submitted with an immunization printout from healthcare provider)

• Certificate of Exemption from Immunizations
